I. Paraneoplastic Autoantibodies
Yo (anti-Purkinje cell antibody)
Indication: cerebellar degeneration
Tumor: breast, ovarian, endometrial, cervical
Clinical features: cerebellar symptoms evolve over weeks to months and then stabilize.
Only one third of patients can walk unassisted.
Neurologic symptoms precede tumor diagnosis in 50%.
Cerebellar degeneration occurs in <1% of the patients at risk.
Pathologic finding: destruction of cerebellar Purkinje cells.
Cerebrospinal fluid (CSF): lymphocytic pleocytosis, increased protein and IgG.
Levels: positive at 1:2,000
Treatment: <10% respond to plasmapheresis, intravenous IgG, steroid, cyclophosphamide.

Indication: sensory neuronopathy, limbic encephalitis
Tumor: small-cell lung cancer, prostate
