Preoperative magnetic resonance imaging (MRI) of the plexus should be done around 5 months of age.

Preoperative electromyography and nerve conduction study can be considered.

Intraoperative physiologic mapping should be planned for in preparation of surgery.

Bipolar needle electrodes are placed in the muscles to be monitored before preparation. Electrodes to be placed in the muscles lying beneath the skin are placed sterilely after preparation. The infant is positioned with a roll underneath the thoracic spine. The roll extends the shoulder. The head is turned toward the opposite shoulder.
