40 A 48-year-old woman developed a left sciatic pain after a fall. She has failed months of conservative therapy. Neuro-logically, she has a positive straight leg raise in the left leg. There is mild spondylitic change with facet hypertrophy at L4–L5 and L5–S1, causing significant left foraminal stenosis at these levels. This is demonstrated on lumbar magnetic resonance imaging (MRI) (Figs. 40-1 and 40-2).
