Sudden inability to form new memory traces (anterograde amnesia) plus retrograde memory loss for preceding days or weeks. Accompanied by bewilderment, anxiety, repeated questions (e.g., “Where am I?”). Immediate registration of events
intact; self-identification preserved. No other abnormality on examination.
